MyChart offers patients personalized and secure on-line access their medical records. It provides a secure method for patients to help manage and receive information about their health.

To request adolescent patient access, the adolescent patient AND their parent/guardian must review, sign, and submit the Informed Consent for Adolescent Patients form.

MyChart Proxy Enrollment

MyChart Proxy access provides an individual who is authorized access to a patient’s records or to perform activities on their behalf (e.g. parent, guardian, patient representative, healthcare surrogate, etc.).

Once access is provided, a Proxy will log into their personal MyChart account and connect to the patient’s information. Please note: a Proxy must have a MyChart account of their own to access the patient information.


Full Access to Child’s MyChart Information

Patients 0 to 11 years + 364 days

Provides parents and guardians full access to all MyChart features through their own MyChart log in. A child age 11-years or younger cannot have a personal MyChart account.

To request full proxy access to a child’s MyChart information, review, sign, and submit the MyChart Proxy Record Request and Acknowledgement of Terms and Conditions form.

Limited Access to Adolescent’s MyChart Information

Patients 12 to 17 years + 364 days

When the child turns 12, MyChart Proxy access is automatically reduced to "limited" for parents and guardians who already had full proxy access. Features such as medications, lab results, and previously sent messages are disabled or reduced due to state laws, which allow for minors' rights to health information privacy. Limited access for parents and guardians includes the ability to:

  • View some vaccination records
  • Schedule Appointments
  • Pay bills

At 12, adolescents may create their own account with full access. To request adolescent patient access, the adolescent patient AND their parent/guardian must review, sign, and submit the Informed Consent for Adolescent Patients form.

Full Access to Adolescent’s MyChart Information

Patients 12 to 17 years + 364 days

Provides parents and guardians full access to all MyChart features through their own MyChart log in. Full proxy access requires consent/acknowledgement by both the patient and the parent/guardian.

Full Access to an Adult’s MyChart Information

Patients 18 years and older

Authorize another person to access your MyChart information to help with your care. Proxy access can be stopped at any time by the proxy or the patient.

To request another adult to have proxy access to your MyChart information, review, sign, and submit the MyChart Record Adult Proxy Release of Information Authorization form.
